These items will need to be signed and returned to me before your child starts in my daycare.
-Medical form complete with TB and lead shots (or note from doctor excluding them)
-Consent forms
-Permission to go on field trips and Permission to take photos for web site
-Application/Record of Child Information
-Robin's Daycare Contract/Payment Agreement
-Deposit to hold spot
-Extra clothes
-Diapers,Wet Wipes ( if needed)
-Meals are included in Daycare fee. I am on a Federally funded Food Program
-New January 2008 - I will need a copy of a certified Birth Certificate
